Stable Supportive Footwear for Self-managing Hip Osteoarthritis Pain — A Randomized Clinical Trial

這項 120 人、6 個月隨機試驗比較「穩定支撐鞋」與「平底柔軟鞋」對髖關節退化疼痛的效果。主要結果未顯示穩定支撐鞋較佳；行走髖痛改善差異為 −0.5 分，信賴區間跨過無效值。臨床重點是：鞋款可作為自我管理選項，但不宜把「支撐性」直接等同於較佳止痛效果。值得注意的是，穩定支撐鞋組不良事件較少。

2. Long COVID：metformin 與 UDCA 治療既有 PASC 未勝過安慰劑

Neither Metformin nor Ursodeoxycholic Acid Effectively Treats Postacute Sequelae of COVID-19 — A Randomized Clinical Trial OA

韓國兩家醫學中心 396 人的雙盲隨機試驗顯示，2 週 metformin 或 UDCA 治療，8 週後 PASC recovery 率與安慰劑相近：metformin 63.6%、UDCA 68.2%、安慰劑 68.2%。這篇的臨床意義是：急性感染期 metformin 可能與 long COVID 預防有關的假說，不能直接外推到「治療已成立的 PASC」。研究也凸顯 PASC outcome measure 與安慰劑／自然恢復效應的挑戰。

3. 高密度乳房補充 MRI：不是「密度高就做」，而是要納入乳癌風險

Outcomes of Density-Targeted Supplemental Breast Magnetic Resonance Imaging Screening by Breast Cancer Risk

這是一篇模擬研究，評估 DBT 乳房攝影加上針對極高乳房密度者的補充 MRI。主要訊息是：對 extremely dense breasts 且乳癌相對風險 ≥2 的族群，補充 MRI 的 harm–benefit ratio 可能接近可接受範圍；但成本效益高度受 MRI 費用與偽陽性切片率影響。臨床上，這篇支持「乳房密度 + 個人風險」而非單靠密度做決策。

4. HPV 疫苗後的子宮頸癌篩檢：可能需要大幅減少頻率

Optimizing Cervical Cancer Screening by Age at Vaccination for Human Papillomavirus

挪威模型研究顯示，對 30 歲前已接種 HPV 疫苗的女性，成本效益較佳的 HPV-based screening 策略通常比現行 5 年一次更稀疏。對 12–24 歲接種者，偏好的策略約為每 15–25 年篩檢一次，一生 2–3 次。這篇的重點不是立即改變所有篩檢政策，而是指出疫苗時代的子宮頸癌風險已變，篩檢制度應開始依「接種年齡與疫苗種類」重新設計。

5. 懷孕早期血紅素：過低與偏高都與新生兒不良結果相關

Hemoglobin Concentration in Early Pregnancy and Severe Neonatal Morbidity and Mortality

6. AF 使用 apixaban/rivaroxaban：合併 diltiazem 比 metoprolol 出血風險高

這篇美國行政資料庫研究比較 AF 患者使用 apixaban 或 rivaroxaban 時，合併 diltiazem 或 metoprolol 的出血風險。配對後每組 23,000 人，diltiazem 與較高 serious bleeding 住院風險相關，且 >120 mg/day 的高劑量風險更明顯。臨床含義：若病人使用 factor Xa inhibitor，rate control 選擇 diltiazem 時要重新評估出血風險、劑量與替代方案。

臨床可帶走的 5 點

  1. 不要把機轉合理等同於臨床有效。 支撐鞋治療髖 OA、metformin/UDCA 治療既有 PASC 都是負向或中性試驗。
  2. DOAC 病人的 rate control 需注意交互作用。 Apixaban/rivaroxaban 合併 diltiazem，尤其高劑量，可能增加 serious bleeding。
  3. 篩檢正走向個人化。 乳癌補充 MRI 要看乳房密度與整體風險；HPV 疫苗後子宮頸癌篩檢可能要依接種年齡下修頻率。
  4. COVID 門診治療重點仍是早期、高風險、抗病毒。 ACP practice points 保留 nirmatrelvir–ritonavir 與 molnupiravir 的角色，同時明確排除 ivermectin 與 sotrovimab。
